Alright, so I used NUS Auto Downloader (great program by the way) to update all of the IOS. I then ran the checker again and here are the changes from the original results, the ones marked * were not originally there before the update.

    IOS11 (rev 257),No,Enabled,Disabled,Disabled,Enabled,Disabled ,Disabled
    IOS20 (rev 257),No,Enabled,Disabled,Disabled,Enabled,Disabled ,Disabled
    IOS30 (rev 2817),No,Enabled,Disabled,Disabled,Enabled,Disable d,Disabled
    IOS50 (rev 5121),No,Enabled,Disabled,Disabled,Enabled,Disable d,Disabled
    IOS60 (rev 6401),No,Enabled,Disabled,Disabled,Enabled,Disable d,Disabled
    IOS61 (rev 5405),No,Disabled,Disabled,Disabled,Disabled,Disab led,Disabled
    IOS70 (rev 6687),No,Enabled,Disabled,Disabled,Enabled,Disable d,Disabled
    *IOS202 (rev 65535),No,Enabled,Disabled,Disabled,Enabled,Disabl ed,Enabled
    IOS222 (rev 65535),No,Enabled,Enabled,Disabled,Enabled,Disable d,Enabled
    IOS223 (rev 4),No,Enabled,Enabled,Disabled,Enabled,Disabled,En abled
    *IOS224 (rev 65535),No,Enabled,Disabled,Disabled,Enabled,Disabl ed,Enabled
    IOS249 (rev 19),No,Enabled,Disabled,Disabled,Enabled,Disabled

The majority of the changes were not to IOS in the list you said needed to be updated. Did I do something wrong?

IDK why, though admittedly I'm unfamiliar with the program I linked you to. There's always this that you can try --- it's more hands-on/manual, less automatic. Go back to my previous reply and select those IOS via NUS Downloader and get'em on your SD card as wads and then install via wad manager.

I think you still need to try new media, to rule out the harddrive and everything to do with the usb port

you can try a sd card and then it might lead you to an ios issue with the usb port

Have you tried a different usb loader?

Just for knowledge, what USb Loader and it's version are you using, and how did you install the ISO's to the HDD.

I'm extracting one of my smaller ISOs now to try to play off of my SD card. And yes, I have tried it with USB loader gx, configurable USB loader and neogamma. Same result all 3 times

How did you install the IOS's, what I'm getting at here is, there was a version of USB Loader GX that would install corrupted ISO's to the USB HDD.

All IOS's were installed through the homebrew channel because I cannot connect my wii to the internet. USB Loader GX rev 921
