Most applications that format drives automatically set it to FAT32 with 64kb cluster size, even if you tell it to do 32kb cluster size. A simple double check will make sure it's right.
Use Acronis Disk Director 11 Advanced to check: right click your disk/change cluster size/current cluster size:
I use Acronis Disk Director 11 Advanced to check my FAT32 hard drive cluster size and it was 64KB so I change it to 32KB and I did not lose any data and it is very quick.
When you check your cluster size, also check and make your partition active and primary
Update to CFG Loader r48 or wiiflow v4.0.1 r431or USB Loader GX R1200,Replace the boot.dol in your SD or USB /apps/ usbloader_gx with the updated one.( r1200_IOS249)
Try from HBC first and if your USB Loaders channels are forwarders you could try the USB Loader channels or install USB Loader GX -UNEO_Forwarder Channel 5_1_AHBPROT.wad for USB Loader GX channel and Default-FIX94v14b-forwarder-DWFA.wad for wiiflow channel.
