To get this working you need:

a Pre-patched copy of the game

    Or

The unpatched (old) release by IND (or any other unpatched release)

If you have the Nitro release (or any other patched release), then all you should need is to install is IOS53-64-v5406.wad

If you have the older version, then you will need to patch it.

    You will need:

    Main.dol
    WiiScrubber 1.4

    Download the required files above.
    Unzip Wiiscrubber 1.40
    Start WIIScrubber
Click Load ISO and load you copy of NSMB.
Look for Main.dol and right click it and selet 'replace', choosing the main.dol you download earlier to replace it with.
Click Scrub, and choose the path where you want to save the fixed ISO.

    Then, GO PLAY!

This is working on Hardmodded Wii's and USB loaders. NOT working on DVD on a softmodded Wii.

    [UPDATE]

    For USB-Loader Users:
    -Install IOS53-v5406
-Try NeoGamma R8Beta7 or OpenWiiFlow, which both should be able to run the Game without any patches needed (Thanks to WiiPower!)
-Or put the fixed ISO on your Drive, using your Favorite Manager, and play it with any Loader
-Or use CFG-Loader and the Alt.dol Trick (rename the fixed .dol to SMNP.dol and place it under /usb-loader/. Then select Alt.dol from SD and play the game)

    For Disc-Loader Users:

-No Confirmation here. Looks like it won't work right now.

    ModChip Users:
-Try NeoGamma R8Beta7, which should be able to run the Game without any patches needed (Thanks WiiPower!)
-Or download and Install IOS53 and your System IOS (3.2-IOS30, 4.0/4.1= IOS60, 4.2=IOS70) and patch them with the Trucha Patch. Use DOP-IOS for this, and play the fixed ISO. You'll need a Wii that is connected to the Internet!

    Neogamma r8 beta7: NeoGammaR8beta7.zip
OpenWiiflow: Downloads - wiiflow - Project Hosting on Google Code
    IOS53-v5406: IOS53-64-v5406.wad
    DOP-IOS: Dop-IOS - WiiBrew

    [SD Card]

    You will need:

An SD-Card (even a 512mb will do for this one!)
A copy of WBFS manager: WBFS Manager - Release: WBFS Manager 2.5
A copy of the game (as above).

    Download this: SD/USB loader WAD
    or
    THIS: Neogamma R7

    Install either with wadmanager (http://teknoconsolas.tv/wanin/WAD-Manager_v1.5.zip)

Now, lets get the ISO on your SD (you can use the same one again now!)

Insert your SD card into your PC.
    Open WBFS manager.
Select your SD card's drive letter in the top left.
    Click Format WBFS.
When it completes, click Load in the top left.
Now move to the right hand side and click browse... find your ISO and load it.
Click Add to drive, and wait for it to complete.

Congratulations! you now have a WBFS formatted SD card with NSMB on it!

Now, insert this SD into your wii, switch the wii on, select the SD/USB loader or Neogamma R7 from your wii system menu, select SD if its not already selected, and PLAY!!
how large of a card will i need? 2gb?nvm!
it still wont work in neogamma i hit A to load and it just flashes

Just follow Dave reccommendations. Bye, Bye green screens, welcome Mario.

    Thanks Dave.

Just reformat your USB with WBFS Manager and run the patch game.

I'm going to wait for the NTSC versions to be released. If all nintendo games have to ran by this method then having a softmod is almost pointless for backup uses.

If you replace it into the iso, as with the first post guide, then thats all you need to do.

If you rename the patched dol and put on the SD, then you dont need to replace it into the ISO, but you WILL have to make sure the SD is kept in the Wii all the time, which is why i preferred the patch and replace method.
that was the only way i could get it to work for me

Eskimo if you have read that you will see that it said that a 512mb card will do.

Is it okay to use my 2gb SD Card that has ALL of my homebrew, backup NAND stuff on it? I see you're formatting the SD, so I'm just a little worried in that regard. Thanks!

no dont do that. If it reformats it all that will be gone. Use a different one or save all that stuff to the computer

So if I save all the stuff to my computer, format the SD card, put the ISO on, and then put all my stuff back on the SD Card (lol, long), it will work? Or is that, like, out of the question?
