I had the same problem but I think I solved the puzzle. The window is asking to press the "A" button. If the "A" button is green I think you may have your gamecube controller plugged in and the game is waiting for you to press the "A" button, the "A" button should be white, this means it is looking for the wiimote. Unplug the gamecube controller should fix the problem.

neogamma4rc2 version worked for me... all the older versions and gamma backup launcher failed to read disc... just do a search for the file. the install the wad

Yes, you are a genius, unplug the GC controller. Anyway this is my setup: Wii 3.2U, cIOS rev10 (wad install), Configurable USB Loader. Normal boot gives green screen, then I change the 002 fix to ON, and all works. It worked for me the first time I installed it, I must of plugged in the GC controller to try out WiiSX PSX emulator (they're starting to work on this again), and forgot to unplug the GC controller. All is fine now.
