Follow this guide. You have to re-soft-mod your wii.
Wii System Menu X(4.1u)-CFG USBLoader 7.0, D2X cIOS Pack

Do you still get a code dump error?
And do you know what IOS you are loading or trying to load from. (Generally it's IOS 249 (for me it is anyway)).

Rev17 is the same no matter what slot it is in, since it can only use IOS38 as a base.

@OP I suggest you try a different loader like Configurable USB Loader and see if that fixes it.

I was going suggest trying to install a different IOS, however if for instance I gave a tut to install to IOS 249 it may not work if the usb loader is not set to boot off IOS 249.

Okay try this - it worked for me with my recent errors and crashing of USB lodaer GX:
Try installing cIOS Installer rev 20b on IOS 249 and then ruunning USB loader. (Network install didn't work for me - you may have to try WAD install - if the file is not on the SD card it will tell you what file you need - just google it and download the wad and place in the sd card root then repeat the rev 20b install choosing WAD install).
If that fails or crashes (or gets a similar code dump) and you have PriiLoader installed make sure you have blocked wii updates.
Next try to perform a wii update from system menu (it will fail) this should then help you with the rev 20b install.

WARNING: YOU SHOULD NOT PROPERLY UPDATE THE WII FIRMWARE - YOU MAY BRICK IT.
