DarkCide has a three part guide on emulators. While I don't necessarily know for sure the answer is in there, did you look? Look in this section...

Edit: of course, as a forwarder this would make it SD card-dependent. No SD card with the rest of the data would also net you back to a restart @ the Wii Menu.

Here is a Wii64 forwarder I made, if you try to load roms from usb, the usb hdd needs to be plugged into usb port 1, that's the usb port not used by a usb loader. Don't laugh, I'm not into graphics. IOS58 is needed to use this forwarder.

    looks to
    SD:/apps/wii64/boot.dol
    USB:/apps/wii64/boot.dol

    Wii64 - N64A.rar

Did you catch my edit earlier after I made my initial reply? Might want to confirm you've got an SD card in the Wii with the rest of the data the forwarder is looking for.

You do not have the app in the correct location on the SD card. And you need IOS58 v6176 installed to your Wii.

Look in the /apps/wii64 folder, is there a file named boot.dol in there?

Wouldn't that be SD:/apps/Wii64/boot.dol? You mention the folder, yet the pathing doesn't show it.

Edit: if memory serves, forwarders rely on what Stomp --- IOS 35 being present? OP: have you checked what IOS you have installed?
