So if I'm on a PAL console I have to use FreeTheWads on the IOS56-64-v5405 from OP? Is there a PAL version available?

Or does it mean that I have to FreeTheWad only the game (if the game is NTSC),...

Yes, I did. I got a little thrown off by the fact that it talked about <long-ass-IOS-thingy> and NTSC.
