Pretty self-explanatory, but I'll help you if I can. The region box is for the event that you get a Pal or Japanese iso, you can change it to USA. The patch update is used only when you see an update in the box. If the update box says "no update" then there is no need to patch. I always change the region first if needed, then do a patch update if there is any kind of entry in the update box. Hope that helps. I never use the Extra box, but it's there if you want it.
