I had this problem doing a friends wii, however I started at 3.4u for software revision and everything went fine with the HBC install and firmware downgrade to 3.2. When i went to install starfall it failed everytime, I ended up reinstalling the cios_fix.wad with custom wad manager and also reinstalling IOS downgrader. then starfall installed. Hope this helps

[STEP 1] [Perform these steps ONLY you are already on 3.3 or 3.4 otherwise Skip to Step 2]

+Load Wad Manager in Homebrew Channel and install IOS16-64-v257-prod.wad
Yes, even if you have received errors trying to install other wads this should still work.

+Load Custom WM in Homebrew Channel and install cios_fix.wad
You cannot install this wad with Wad Manager. This is a custom wad manager designed to work for IOS16.

+Load IOS Downgrader in Homebrew Channel. Just follow any prompts until it tells you it is complete.
**Note: This is NOT the same as the cIOS downgrader. The cIOS downgrader allows you to upgrade or downgrade to the ideal firmware
version which is 3.2. The reason 3.2 is so popular is partially because versions after this one had the Trucha bug fixed, and almost
as importantly ver 3.2 is required to run Starfall. Starfall is important because it is at this time one of the only ways to fix a
partially bricked Wii. It is also handy for skipping disk updates.

Is that what you are saying, because I did that. >_>

I did it as well but for some reason after successfully downgrading to 3.2u starfall wouldn't install, so i reinstalled the ios' that I mentioned above and then starfall installed fine. Just thought you might want to try reinstalling the cios_fix.wad and the IOS downgrader. Not the CIOS downgrader that you select the 3.2u revision from. the downgrader you install to make that one work. It worked for me, I am pretty new to this and have only done 3 wii's thus far so maybe wait for someone with more experience to validate that reinstalling those .wad's won't be harmful

TBH... it is very ... let me see... idiotic due to the fact it takes you ages to get the combo spot on, hint : Press them exactly at the same time. then it should register it and let you in.
Here to help in anyway shape and or form (or at least try, ive been out of the wii scene for quite a while)

When you've tried this for more than several minutes and still not working, I had the same problem. In this forum i received help and what you need to do is run IOS downgrader over again. That worked for me as I've downgraded 3 Wii's, and this happened to me on 1 downgrade. I was stumped until i went back to the step where you run the IOS downgrader. Trust me...... It will work.
