Are you using DVD-R, the best are Verbatum. Just a quick scan of your data I see,
    I 14:59:10 Destination Media Type: DVD+R (Disc ID: MBIPG101-R05-01) (Speeds: 3.3x, 6x, 8x)
Not sure if it thinks its a DVD+R.

You need to use DVD-R. I saw a line in your text that said DVD+R. That may be a red herring, are you useing DVD-R. Use image burn at a slow rate.

You should use image burn at less that x4. Nero or something similar works.
